Injuries and possible Insurance procedure
It is important at the beginning of the season when you attend the initial training sessions that you contribute towards your club fees (ie. training session money). This in part goes towards your insurance for any injuries that are covered on the club insurance policy.
If a member gets injured, an accident report form is required to be completed as soon as possible (on the same day if you can). Captains will have copies for you and spares in the lock up. You must also inform Etone Sports Centre as soon as it happens. Etone will also have an accident report form that will require completing.
If you require hospital assistance please ask for some
documentation to confirm you attended hospital.
If you are unable to work and you need to see your doctor/dentist,
then also photocopy any paperwork (sick notes etc) as it may be necessary to send
in the originals - if the paperwork is not clear enough for the insurers to
read. They will return them if you ask
for them back.
For the first 7 days away from any work place this will not be covered
- obviously depending on the nature of the injury and what the insurers say.
Document further appointments concerning the incident/accident and ask for any
witnesses to make a statement – ie. to say what they witnessed. On the claim form it does ask for witnesses’
names and addresses (all information is protected by the Data Protection
Act). This information will be useful
when the paperwork comes from the insurance company (Perkins Slade) as they will also ask if
there were any witnesses, and you will already have this information. On the claims form there is a part that says
to be filled in by a General Practitioner - sometimes your Doctor may
charge for this.
It is advise able to take photocopies of all paper work
relating to the incident/accident. Basically
copy and record everything it makes the process a lot easier.
